GENERAL CONDITIONS
General Terms and Conditions of the Poliklinika Smile determine the way of business, instructions, rights and obligations of patients and providers (Poliklinika Smile), payment methods, discounts and other provisions in order to inform patients better.
TERMS AND CONDITIONS OF PAYMENT
The services of the Poliklinika Smile can be paid in cash, banknotes in Croatian Kuna or in foreign currency, with the previous change in exchange offices or banks.
In addition to cash, it is also possible to pay by cash or credit card, debit card, bank transfer or credit offered in cooperation with Privredna banka Zagreb.
The patient is obliged to pay the full bill for services performed immediately after the end of therapy, after leaving the dental office at the reception or at the billing office.

BOOKING AN APPOINTMENT FOR SURGICAL PROCEDURES
Each patient is obliged to make a payment of a non-refundable deposit of 20% of the total amount of the offer in order to book an appointment for surgery procedure, not later than 7 days before the agreed term of the surgery. Payment can be made by wire transfer to IBAN Poliklinika Smile, HR02 2340 0091 1108 2750 9, SWIFT PBZGHR2X, Privredna banka Zagreb d.d., by calling 00, or in person, by visiting Poliklinika Smile. The rest of the amount of the confirmed offer (for contracted procedure) must be paid no later than 48 hours before the booked apointment of the surgical procedure if the patient wants to obtain a discount for the advance payment.
Surgery involves all implant and surgical procedures under local or general anesthesia.
If the patient is unable to reach the booked appointment of the scheduled surgery for justified reasons, and has timely informed the Poliklinika Smile no later than 48 hours before the scheduled surgery, the patient can use the amount of the paid deposit for the same surgery within six months from the day of cancellation. The same can be exchanged for other services of the Poliklinika Smile. If the patient withdraws from the scheduled surgery and does not inform the Poliklinika Smile, or inform the Poliklinika Smile less than 48 hours before the agreed term, the paid deposit of 20% of the total amount of the offer will not be returned to the patient and the possibility of scheduling a new appointment is not given. for the service from which he gave up, as well as the possibility of replacement for other services of the Poliklinika Smile.
TERMS AND CONDITIONS OF THE PRE-PAID SERVICE AND SERVICE PACKAGES
The patient is obliged to use the amount of prepaid services of the Poliklinika Smile within one (1) year from the date of the offer. If the patient does not use all the services from the prepaid package within the aforementioned period, he is not entitled to a refund for services that he did not use. If the patient is not able to use all the services in the paid package for justified reasons, the Poliklinika Smile gives the patient the opportunity to use other services provided by the department for which the treatment / service package has been paid or to transfer the unused services within 1 year from the date of invoicing. for use by a third party. The third party has the right to use the treatments in the remaining amount not used personally by the patient who paid the package of treatments / services within the department where the package was paid and within the remaining period of one year, with the written permission of the patient with advance payment.
If the patient does not wish to use the paid package for any of the above options, the patient is not entitled to a refund for unused treatments / services and they are considered to be fully utilized. If the patient fails to cancel the prepaid treatment / service within 48 hours before the agreed term, the agreed treatment / service is considered to have been duly performed.
It is particularly important to note that the price of treatments / services that make up an integral part of a package is determined at a preferential amount relative to the price of those treatments / services outside the package, and the price of those outside the package is calculated at the price for that treatment / service in the unit price. indicated in the valid price list of the Poliklinika Smile in the month of purchase of the package. Poliklinika Smile guarantees the quality of every treatment and service provided, however, due to the uniqueness of the human body and the individual reactions of each organism, there is a possibility that the desired results may not be achieved or to a lesser extent, especially if the patient does not adhere to all steps that are required to achieve the goal of the treatment and / or service performed and with which he or she was previously aware. In this case, the patient is not entitled to a refund of money paid for treatments and / or services.
Please note that the funds from the advances paid were immediately and promptly engaged in order to procure the necessary materials necessary to perform the agreed procedures of each individual patient who paid the advance.
The down payment will fully cover all further agreed works and interventions for which it has been paid, and that, in agreement with each individual payer, it can be used, with the payer's written consent, solely for performing the procedure on other patients or family members of the payer patient who is advance payment if the material purchased is compatible with those works and interventions to be performed on another patient to whom the right to use the advance is transferred.
Patients who have made a down payment are also informed that it will not be possible to meet the requirements for a down payment as the works and procedures that are contracted and paid will be completed as soon as the health conditions are met, as already stated. immediately used to cover the procurement of material costs for each individual payer.
INSTALLATION OF STRAUMANN DENTAL IMPLANTS
Each Straumann implant is warranted against a material to demonstrate the absence of any impurities of the material, and the guarantee and originality of the implant are proven with the help of a Straumann passport with a unique serial number for each implant implanted. The Poliklinika Smile replaces the non-integrated implant at the expense of the Piliklinika Smile exclusively for the patient who had the implant implanted at the Poliklinika Smile, provided that he complied with all instructions of the oral surgeon at the Poliklinika Smile and came to regular post-operative controls.
Straumann has a lifetime warranty on the implant and a 10year warranty on prosthetic components (abutment and screw).
IMPLANT PROSTHETICS (CROWNS, BRIDGES)
If there is a dental implant failure, prosthetic replacement should be performed. Considering the reasons for the implant failure will depend on the fact who bears the cost of prosthetics. If the failure of the implant occurred within 90 days from the day of prosthetic treatment, the cost of new work shall be borne by the Poliklinika Smile.
If the failure of the implant is due to failure due to poor hygiene, lack of controls and similar non-compliance with the instructions of the specialist of the Poliklinika Smile and reasons that could not be influenced by the Poliklinika Smile, the cost of the new work is borne by the patient.
PROSTHETICS ON YOUR OWN TOOTH (CROWNS, VIRUSES, BRIDGES)
If the failure of prosthetic work occurred immediately after prosthetic treatment, the cost of new work is borne by the Poliklinika Smile.
If the failure of the implant is due to failure due to poor hygiene, lack of controls and similar non-compliance with the instructions of the specialist of the Poliklinika Smile and reasons that could not be influenced by the Poliklinika Smile, the cost of the new work is borne by the patient..

PROSTHETIC WORK GUARANTEES
All metal ceramic crowns, excluding metal crowns, veneers, wironite denture are guaranteed for 2 years in case of cementation and failure and therefore rupture of the crown. The acrylic denture also has a 2year warranty if it is a permanent denture. Retention elements such as retention clips or prosthesis anchors have a 2-year warranty. The guarantee does not cover temporary crowns, bridges and mobile dentures as temporary solutions.
GUARANTEES ON RESTORATIVE AND ENDODONTIC TREATMENTS
All composite fillings have a 1year warranty, while ceramic fillings (inlay, onlay, overlay) have a 2year warranty.
There is no guarantee of root canal filling and revision of root canal because of the high risk due to individual variations in root canal morphology.
THE WARRANTY IS NOT APPLICABLE IF:
• the patient does not adhere to the oral hygiene provider's instructions,
• the patient does not maintain hygiene with appropriate products as recommended by the provider,
• the patient improperly uses or maintains mobile prosthetic restorations,
• the patient does not come to regular check-ups at least once every 3 or 6 months depending on the assessment and guidance of the provider,
• the patient does not accept the therapy to be performed in case the control determines the possibility of developing diseases that would compromise work,
• the patient is diagnosed with a disease of the tooth or surrounding tissue after completion of the provided health care, which requires treatment that requires complete removal or partial damage (e.g. periodontal disease, pulpitis, cyst, periapical process) that is not visible during therapy,
• there is a general illness that affects the patient's masticatory system (e.g. diabetes, epilepsy, osteoporosis, chemotherapy, radiation in oncology patients),
• the patient significantly loses or gains weight over a short period of time,
• damage occurs during incidents or emergencies (e.g., fall, impact, scuffle, crash),
• damage caused by poor eating and other habits,
• damage occurs due to circumstances that could not be affected by the Poliklinika Smile
DISCOUNTS AND ACTIONS
Poliklinika Smile occasionally offers special discounts to promote new products and / or services. Discounts are limited in time with the start date and end date of the action. In addition to the time limit, discounts have a clearly indicated percentage discount with the announcement of the special (reduced) price and the regular price. Discounts cannot be added together and do not apply during the course of the action for already discounted products or services. Each action clearly states the conditions for obtaining the right to a discount as well as the payment methods to which it applies.
